Suzanne Gazda M.D will direct-bill your insurance provider, exclusive of any applicable co-pays, for patient visits and related services (labs, etc.). We accept most major insurances and plans. To verify specific insurances accepted or if you do not have insurance coverage, please contact our office with any questions.
Costs for various treatments and other testing that may not be covered by your insurance provider are available on our Forms section.
Cancellation and Other Policies
While we understand unforeseen circumstances may arise, please provide 48 hours notice if you are unable to keep your appointment so that we may accommodate other patients who are waiting to be scheduled.
